- Signed Translation Certification Statement including date, statement of accuracy, and full translator's name and address
- Complete translation of any text that is not in English. Headers, stamps, seals, handwritten notes, and marginal annotations must all be included. Partial translations or extract translations are not accepted.

- Notarized Translation
- Physical version of the translation
- Original (source) document
