When Do You Need This Form?

A minor travel consent form (also called a child travel authorization letter or parental consent form) is a document that grants permission for a child to travel without both parents present. While not always legally required, this form can prevent delays at airports, border crossings, and cruise terminals.

You need this form when your child is traveling with only one parent while the other parent stays home. Many countries and airlines will ask for proof that the absent parent consents to the travel, especially for international trips.

The form is also essential when your child is traveling with grandparents, aunts, uncles, or other relatives. Without proper documentation, these family members may face questions from authorities about their relationship to the child and their authority to travel with them.

Cruise lines almost universally require consent forms for minors traveling without both parents. International travel to countries like Mexico, Canada, Brazil, Dominican Republic, Colombia, and the Philippines often requires notarized parental consent.

How Is This Different From a DS-3053?

The DS-3053 (Statement of Consent) is a U.S. Department of State form used specifically when applying for a passport for a child under 16 and one parent cannot appear in person at the passport acceptance facility. It must be notarized and submitted with the passport application (Form DS-11).

A minor travel consent form, on the other hand, authorizes a child to travel domestically or internationally without one or both parents. It is used at airports, cruise terminals, and border crossings to demonstrate that the absent parent has given permission for the trip.
